The State provides low-cost health coverage to children in families that earn too much money to qualify for Medicaid.  The program is called Healthy And Well Kids in Iowa program (Hawki).

Learn about Hawki Children’s Health Insurance

Your family size and income determines the monthly cost.  Some families pay nothing at all, and the maximum any family pays is $40 per month. Individuals must also meet eligibility requirements

Families choose between one of two Managed Care Organizations (MCO):  

Amerigroup Iowa, Inc.

Iowa Total Care

Dental benefits for those who qualify for Hawki are covered by Delta Dental of Iowa.

Apply for Hawki Insurance Online, by Paper Application, or by Phone

You can apply online with the HHS benefits portal Hawki application.

OR

You can apply with a paper application.  You can get the paper application by:

  • Visiting  the Health and Human Services website to print the paper Hawki application.
  • Asking for the application to be sent to you. Call HHS Contact Center and ask for an application to be mailed to you. Medicaid: 1-855-889-7985, Hawki: 1-800-257-8563.
  • Picking up an application from your  local HHS Office. Office Locator 

Complete the form and be sure to sign the application.

Return the form to a local HHS office, or email it to imaginingcenter4@dhs.state.ia.us, or fax it to 515-564-4016.

You can apply by phone by calling the HHS Contact Center at 1-855-889-7985

A representative will ask you questions and walk you through the application.

The representative will get a verbal signature from you and submit the application on your behalf.

Wait for Your Hawki Application to be Processed

It can take up to 45 days to process an application.

Respond to requests for information (RFIs). If you receive RFIs in the mail respond to HHS with the requested information by the given due date to ensure your application is processed.

You will receive a Notice of Action in the mail when your application has been processed letting you know the outcome of your application.
